MarketEdge Bonus Index Product Disclosure form change
The generic version of MarketEdge Bonus Index Product Disclosure (ET-MEDG-1101) has been updated with the surrender charge variation for MN and PA. The form date on the new forms is 08-25. Please use the updated version of the Product Disclosure for all states — except NJ and WI, which have state-specific variations. Note: There are state-specific Disclosure and Comparison forms for MN (ET-MEDG-4801MN) and PA (ET-MEDG-4801PA).

Updated distribution forms available
D istribution forms have been updated to include required revisions to W-4 language. Please ensure you’re using the most current version of all forms to avoid processing delays. Outdated versions will no longer be accepted after October 31, 2025.
Friendly reminder — Use the appropriate Income Benefit Rider (IBR) form
To help ensure timely processing and a smooth experience for your clients, please download and submit the correct IBR based on the client’s contract and eligibility.
Income Benefit Rider Service Request Form: ET-2703 (04-25) Use this form to request Income for Life under the standard IBR. Note: This is the most commonly used form.
Enhanced Income Benefit Rider (ENHIBR) Service Request Form: ET-ENHIBR (04-25) Use this form only if the client’s contract includes the ENHIBR Rider and the client has been certified as chronically ill by a physician.
MarketEdge Bonus Index issued on Contract Form Series ICC19-ET-MP10-2000(02-19) or ET-MP10-2000(05-18). Riders issued on Form Series ICC18-430-NHW(06-18) or 430-NHW(08-03); ICC16-ET-TI(10-16) or ET-TI(10-16); and ET-AVBR(06-09).
