On a client’s contract anniversary, they’ll receive a transfer form that shows only the index account options available for their specific contract.

At Agents.EquiTrust.com under Statements & Letters for each contract.

How will clients get it: 

The contract-specific transfer form will be mailed to clients in place of the previous generic version with their renewal notice (30 days before their anniversary) and their annual statement.
